Sept 6 COVID-19 Vaccine Policy Compliance Deadline

Dear students,

The deadline to upload your COVID-19 vaccination record or to submit an exception or deferral request is fast approaching. Please provide your information by September 6.

Vaccine documentation can be submitted at the UCI Student Health Center Patient Portal:osh.chs.uci.edu.

As a reminder, students who fail to upload vaccination verification or exception or deferral request are subject to:

  • Loss of access to UCI’s Wi-Fi system, UCI Mobile Access
  • Registration hold that restricts their ability to register for classes for the Winter 2022 quarter
  • Noncompliance notifications that will result in a referral to the Office of Academic Integrity and Student Conduct for disciplinary action, including, but not limited to, probation, suspension or dismissal

Exception and deferral forms, and instructions for submitting them to the Student Health portal, can be found online: shc.uci.edu/immunization-requirements/exemptions.

Those who submit their vaccine information or exception/deferral by August 27, 2021 will be eligible to win priority enrollment for Winter and Spring quarters this academic year, or a gift card from UCI’s bookstore, The Hill.

For those of you who already submitted your COVID-19 record or your exception or deferral request, we appreciate your commitment in supporting our campus efforts to start the school year more safely. Once your exception or deferral request is reviewed, you will receive a message containing the decision and next steps. If you will have only received your first dose of a two-dose COVID-19 vaccine by September 6, please upload your first dose documentation now, and your second dose as soon as you receive it.

CTVNS can also assist international students with issues related to vaccine documentation and can answer questions as to whether vaccines received internationally meet the UC COVID-19 Vaccine policy requirements.
